This professional dog walking registration form is ideal to use to enrol a new dog walking client.  It is an editable dog walking registration form that you can update in Word as required.  In addition, you can print off the PDF version.

  • WORD - Microsoft Word file (compatible with 2010 and higher version) -MUST USE WORD TO EDIT
  • Created using a table in Microsoft Word – you should be proficient in Word to customize this form.
  • Any checkboxes may not be clickable if using an older version of Word.
  • PDF file (ready to print)
  • PDF to be used as a printable but you can use software such as PDFEscape to make it fillable.



  • Word document (.docx) – MUST USE WORD TO EDIT - should be proficient in WORD to update this (2 pages)
  • PDF Printable document to allow you to print a copy without a logo (2 pages)

This form uses Word tables which may be difficult for an inexperienced Word user to change but changing the text should be straightforward. If using Google Docs the paws watermark will be too dark as it is not supported.
